Lease/Rental Toward Purchase
Modifier LL identifies durable medical equipment furnished under a lease or rental agreement in which rental payments are applied toward the equipment's purchase price.

What Is Modifier LL?
Modifier LL is a HCPCS Level II modifier used to identify durable medical equipment (DME) furnished under a lease or rental arrangement in which rental payments are applied toward the purchase price.
Its official descriptor is “Lease/rental (use the 'LL' modifier when DME equipment rental is to be applied against the purchase price).”
Unlike a standard rental arrangement, an LL agreement provides for eligible rental payments to count toward the eventual purchase of the equipment.
Modifier LL communicates this payment arrangement to the insurer. It does not independently establish coverage or authorize a lease-to-own transaction.
Important: Not every DME rental qualifies for LL. Medicare and other payers may have specific restrictions on lease-to-purchase arrangements.
Quick Reference
Modifier: LL
Type: Payment Arrangement
Category: DMEPOS
Meaning: Lease/Rental
Billing: Toward Purchase
Medical Necessity: Required
When to Use Modifier LL
Lease-to-Purchase Agreement: Report LL when durable medical equipment is furnished under an eligible rental agreement that applies rental payments toward the purchase price.
Rental Payment Credits: Use LL when the payer recognizes the rental payments as credits against the equipment's eventual purchase.
Eligible DME Codes: Append LL only to equipment codes that support the applicable lease/rental billing arrangement.
Payer-Approved Arrangement: Confirm that the insurance plan permits lease-to-purchase billing before reporting LL.
Documented Purchase Terms: Maintain an agreement identifying the rental payments, purchase price, and how credits are applied.
Important: LL identifies the payment arrangement, not the equipment's condition or the number of rental months.
When NOT to Use Modifier LL
Standard Equipment Rental: Do not report LL when rental payments are not applied toward a purchase. Modifier RR may be appropriate instead.
Immediate Equipment Purchase: LL is not intended for equipment purchased outright without a preceding rental arrangement.
Unsupported Lease Agreement: Do not use LL when the payer does not recognize rental payments as purchase credits.
Routine Capped Rental Billing: Do not assume Medicare capped rental equipment requires LL simply because ownership may transfer after the rental payment period.
Equipment Repairs or Maintenance: LL does not identify repair, replacement, or maintenance services.
Important: The existence of a rental agreement alone does not establish eligibility for Modifier LL.
Modifier LL vs. RR and NR
These modifiers describe different aspects of DMEPOS rental and purchase billing.
Modifier LL – Lease/Rental Toward Purchase: Identifies an eligible arrangement in which rental payments are applied toward the equipment's purchase price.
Modifier RR – Rental Equipment: Identifies equipment furnished on a rental basis without establishing that payments are credited toward a purchase.
Modifier NR – New When Rented: Identifies equipment that was new when originally rented and is subsequently purchased under an applicable billing arrangement.
Key Difference: LL identifies the financial arrangement, RR identifies rental status, and NR identifies the equipment's original condition when a qualifying rental transitions to a purchase.
Important Billing Rule: These modifiers are not interchangeable. Select the modifier required for the equipment transaction and payer's billing policy.
Billing Example
A patient requires a hospital bed for home use. Their insurance plan permits a lease-to-purchase arrangement in which monthly rental payments are credited toward the equipment's purchase price.
The supplier provides the hospital bed under an approved agreement and bills the qualifying rental payments using Modifier LL.
Example – Hospital Bed Lease-to-Purchase:
E0260-LL
Claim Details:
HCPCS Code: E0260 – Hospital bed, semi-electric, with mattress
Modifier LL: Lease/rental toward purchase
Payment Arrangement: Monthly rental payments credited toward purchase
Equipment: Semi-electric hospital bed
Documentation: Lease agreement and payment records
Illustrative Payment Calculation:
Equipment purchase price: $1,200
Monthly rental payment: $100
Rental payments credited after four months: $400
Remaining purchase balance: $800
This example illustrates how a lease-to-purchase arrangement may operate. It does not represent a Medicare payment schedule or establish that E0260 qualifies for LL under every payer.
Important: The supplier must verify that the payer recognizes the arrangement, accepts LL for the HCPCS code, and specifies how rental payments are applied toward purchase.
Documentation Requirements
Modifier LL claims require documentation supporting the medical necessity of the equipment, the lease-to-purchase agreement, and the application of rental payments toward the purchase price.
Equipment Order: Maintain a valid written order identifying the prescribed equipment.
Medical Necessity: Document the patient's condition and the clinical need for the equipment.
Lease Agreement: Retain the agreement describing the rental terms, purchase option, and payment obligations.
Purchase Price: Document the agreed purchase price and any applicable payment limitations.
Rental Payment History: Maintain records showing rental payments made and amounts credited toward purchase.
Ownership Terms: Identify the conditions under which ownership transfers to the beneficiary.
Documentation Checklist:
Valid equipment order retained
Medical necessity established
Lease-to-purchase agreement documented
Purchase price verified
Rental payment credits recorded
Payer requirements confirmed
Important: The agreement and payment records must support the lease-to-purchase arrangement reported on the claim. A purchase option alone does not establish that rental payments qualify as purchase credits.
Lease-to-Purchase Billing Rules
Modifier LL requires particular attention to rental payment credits, ownership transfer, and payer-specific reimbursement requirements.
Rental Credits: The agreement must establish how qualifying rental payments reduce the equipment's purchase price. The credit calculation must follow the payer's applicable rules.
Ownership Transfer: Determine when the beneficiary becomes the equipment owner. Ownership does not necessarily transfer after a fixed number of rental payments.
Purchase Balance: Verify the remaining amount owed after eligible rental credits and ensure the claim does not exceed applicable payment limits.
Medicare Capped Rentals: Standard Medicare capped rental equipment follows established monthly payment and ownership-transfer rules. These arrangements should not automatically be reported using LL.
Modifier NR: If equipment was new when originally rented and is subsequently purchased, NR may be relevant to the purchase transaction. It does not replace LL's identification of a lease-to-purchase arrangement.
Payer Differences: Medicaid and commercial insurers may establish separate rules for lease agreements, purchase credits, modifier combinations, and ownership transfer.
Important Billing Rule: Modifier LL identifies a rental arrangement with purchase credits. It does not independently permit additional reimbursement beyond the equipment's applicable payment limits.
Common Billing Mistakes
Claims involving Modifier LL may be denied when the lease arrangement, payment history, or equipment eligibility does not satisfy payer requirements.
Standard Rental Reported as Lease: Using LL when rental payments are not credited toward purchase.
Unsupported Equipment Code: Reporting LL with a HCPCS code that does not qualify for the payer's lease-to-purchase arrangement.
Missing Lease Documentation: Failing to retain an agreement establishing the purchase price and rental credit terms.
Incorrect Payment Credits: Reporting a purchase balance that does not properly account for previous rental payments.
Duplicate Reimbursement: Billing rental or purchase amounts already reimbursed under the equipment's payment arrangement.
Incorrect Ownership Status: Continuing lease payments after ownership has transferred when additional rental billing is not permitted.
Medical Necessity Not Established: Documentation does not support coverage of the prescribed equipment.
Important: Review the lease agreement, prior payments, payer policy, and remittance advice before correcting or appealing a denied claim.
Frequently Asked Questions
Can Modifier LL be used with Modifier RR?
Only when the payer's billing instructions permit or require the combination. LL identifies a lease/rental arrangement with payments applied toward purchase, while RR identifies equipment furnished on a rental basis. Do not automatically append both modifiers.
Does Modifier LL guarantee that the patient will eventually own the equipment?
No. Ownership depends on the lease agreement, payment requirements, and applicable payer rules. LL identifies the payment arrangement but does not independently guarantee ownership transfer.
