What Is Modifier KJ?
Modifier KJ is a HCPCS Level II modifier used to identify later rental months for qualifying durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS).
Under Medicare's capped rental payment system, KJ generally identifies rental months four through thirteen. It follows Modifier KH, which identifies the initial rental month, and Modifier KI, which identifies the second and third rental months.
Modifier KJ is commonly reported with Modifier RR to indicate that the equipment is being rented.
The modifier also applies to certain parenteral and enteral nutrition (PEN) pump rentals, which have different payment rules from standard capped rental DME.
Important: Modifier KJ identifies the applicable rental period. It does not independently establish medical necessity, coverage, or eligibility for reimbursement.
Quick Reference
Modifier: KJ
Type: Rental Period
Category: DMEPOS
Meaning: Rental Months 4–13
Billing: Months 4–13
Medical Necessity: Required
When to Use Modifier KJ
Fourth Rental Month: Begin reporting KJ when qualifying capped rental equipment reaches its fourth rental month.
Months Five Through Thirteen: Continue using KJ for subsequent eligible rental months under the standard capped rental payment system.
Capped Rental Equipment: Report KJ with eligible HCPCS codes subject to Medicare's capped rental payment rules.
Rental Billing With RR: Append KJ alongside Modifier RR when both are required to identify the rental transaction and billing period.
Qualifying PEN Pump Rentals: Use KJ for applicable later rental months of parenteral and enteral nutrition pumps according to their separate billing requirements.
Continued Medical Necessity: Confirm that the beneficiary continues to qualify for the equipment under applicable coverage policies.
Important: KJ must correspond to the actual rental period and the equipment's payment category.
When NOT to Use Modifier KJ
First Rental Month: Do not use KJ when Modifier KH is required to identify the initial capped rental claim.
Second or Third Rental Month: Use Modifier KI rather than KJ during these rental months.
Equipment Purchases: KJ does not identify purchases of new or used equipment. Modifiers NU or UE may apply instead.
Ineligible Equipment Categories: Do not automatically report KJ for equipment governed by different Medicare payment rules.
After the Capped Rental Period: Do not continue routine capped rental billing beyond the applicable payment limit unless a specific Medicare rule permits additional payment.
Incorrect Rental History: Do not use KJ to restart or extend a rental period without an eligible billing circumstance.
Important: Verify the rental month, equipment category, and applicable payment limitations before reporting KJ.
Modifier KJ vs. KH and KI
Modifiers KH, KI, and KJ identify different stages of qualifying DMEPOS rental arrangements.
Modifier KH – First Rental Month: Identifies the initial month of a qualifying capped rental period.
Modifier KI – Second and Third Rental Months: Identifies the next two months of the rental arrangement.
Modifier KJ – Fourth Through Thirteenth Rental Months: Identifies later months of standard capped rental equipment.
Key Differences:
KH: First rental month.
KI: Second and third rental months.
KJ: Fourth through thirteenth rental months.
Important Billing Rule: Modifier RR identifies the equipment rental, while KH, KI, and KJ identify the applicable rental stage. PEN pump billing may follow different rental-month limits.
Billing Example
A Medicare beneficiary receives a continuous positive airway pressure (CPAP) device under a qualifying capped rental arrangement. The supplier has completed billing for the first three rental months and is preparing the fourth monthly claim.
Because the equipment has entered its fourth rental month, the supplier reports Modifier KJ with Modifier RR.
Example – Fourth-Month CPAP Rental:
E0601-RR-KJ
Claim Details:
HCPCS Code: E0601 – Continuous positive airway pressure (CPAP) device
Modifier RR: Rental equipment
Modifier KJ: Fourth rental month
Billing Period: Month 4
Rental Billing Sequence:
Month 1: E0601-RR-KH
Months 2–3: E0601-RR-KI
Months 4–13: E0601-RR-KJ
Important: CPAP claims may require additional modifiers, including KX when applicable coverage criteria are met. Continued payment depends on satisfying Medicare's equipment-specific coverage and documentation requirements.
Documentation Requirements
Claims involving Modifier KJ must be supported by documentation establishing continued medical necessity, the correct rental period, and compliance with applicable Medicare coverage requirements.
Valid Equipment Order: Maintain the required written order supporting the prescribed equipment.
Medical Necessity: Ensure the medical record supports continued coverage under the applicable equipment policy.
Proof of Delivery: Retain documentation confirming that the equipment was originally furnished to the beneficiary.
Rental History: Verify that the first three rental months have been accounted for and that the current claim falls within the applicable KJ billing period.
Continued Use: Maintain documentation supporting continued equipment use when required by the relevant Medicare coverage policy.
Coverage Requirements: Confirm that any equipment-specific follow-up evaluations, testing, or other conditions for continued payment have been satisfied.
Documentation Checklist:
Valid equipment order retained
Medical necessity supported
Correct HCPCS code selected
Rental month verified
Proof of delivery available
Continued-coverage requirements satisfied
Important: Documentation must support both the equipment's continued eligibility for payment and the rental period identified by Modifier KJ.
Billing and Claim Considerations
Modifier KJ should be reported on the same claim line as the applicable HCPCS equipment code when identifying later rental months of qualifying DMEPOS equipment.
Modifier RR Combination: Report KJ with RR when both modifiers are required for the rental claim.
Rental Month Tracking: Maintain accurate billing records to ensure KJ is used only during the applicable rental period.
Medicare Payment Calculation: For standard capped rental DME, Medicare generally pays a lower monthly rental amount beginning in month four than during the first three months. Actual reimbursement depends on the applicable fee schedule and payment rules.
Thirteenth-Month Limit: Under standard capped rental rules, ownership generally transfers to the beneficiary after the thirteenth paid rental month. Additional routine rental claims are generally not payable after the capped rental period ends.
PEN Pump Exceptions: Certain parenteral and enteral nutrition pumps have different rental payment arrangements. Suppliers must follow the applicable equipment-specific rules rather than assuming the standard 13-month limit applies.
Additional Modifiers: Apply other required coverage or billing modifiers, such as KX, when supported by the equipment's Medicare policy.
Payer Requirements: Commercial insurers and Medicaid programs may have different rental limits, modifier requirements, and payment arrangements.
Important Billing Rule: Confirm the equipment's payment category, rental history, and applicable coverage requirements before submitting a KJ claim.
Guidelines and Common Billing Mistakes
Incorrect use of Modifier KJ can result in claim denials, payment delays, or inaccurate rental reimbursement.
Using KJ Too Early: Reporting KJ during the first three rental months instead of KH or KI.
Billing Beyond the Rental Limit: Continuing standard capped rental claims after the applicable payment period ends.
Missing Modifier RR: Omitting RR when required to identify the equipment rental.
Incorrect Rental History: Reporting KJ without verifying previous rental payments and the current billing period.
Using KJ on Purchases: Appending KJ to equipment purchased rather than rented.
Ignoring Equipment-Specific Rules: Applying standard capped rental limits to equipment with different payment arrangements, including certain PEN pumps.
Important: Verify the rental month and applicable equipment payment rules before submitting each claim.
Common Denial Reasons
Claims involving Modifier KJ may be denied when rental history, modifier combinations, or continued-coverage requirements do not satisfy payer guidelines.
Incorrect Rental Month: KJ is reported before the fourth qualifying rental month or outside the applicable rental period.
Rental Limit Exceeded: The claim exceeds the permitted number of rental payments.
Invalid Modifier Combination: Required modifiers are missing or the reported combination is unsupported.
Medical Necessity Not Established: Documentation does not support continued coverage of the rented equipment.
Duplicate Rental Claim: A claim has already been processed for the same equipment and billing period.
Continued-Coverage Requirements Not Met: Required follow-up evaluations, usage evidence, or other documentation are missing.
Important: Review the remittance advice, previous rental claims, and applicable Medicare coverage policy before correcting or appealing a denied claim.
Frequently Asked Questions
Does Modifier KJ have the same rental limit for all equipment?
No. KJ generally identifies months four through thirteen for standard Medicare capped rental DME, but certain PEN pumps follow different rental arrangements. Suppliers must verify the payment category and rules for the specific HCPCS code.
Does replacing rented equipment restart the KJ rental period?
Not automatically. Routine repairs, exchanges, or supplier changes generally do not establish a new capped rental period. A new rental period may be permitted under specific Medicare replacement rules when the applicable conditions are satisfied.
