What is Modifier AZ?
Modifier AZ identifies a physician service furnished in a dental Health Professional Shortage Area (HPSA) for purposes of an Electronic Health Record incentive payment.
CMS created Modifier AZ effective January 1, 2011 so eligible professionals could identify qualifying services performed in a dental HPSA when the service ZIP code did not fall completely within the designated dental HPSA.
The modifier was connected to Medicare's EHR Incentive Program and the additional incentive available to qualifying eligible professionals furnishing a sufficient portion of their services in HPSAs.
