Modifier 81- Minimum Assistant Surgeon

Modifier 81 is used to identify a physician who provides minimal assistant surgeon services during only a portion of a surgical procedure

What is Modifier 81?

Modifier 81 indicates that a physician provided minimal surgical assistance to the primary surgeon during an eligible procedure. Unlike Modifier 80, which is reported when the assistant surgeon participates throughout most or all of the operation, Modifier 81 is appropriate when assistance is needed for only a limited portion of the procedure. This modifier is reported by the assistant surgeon when supported by medical necessity and payer guidelines.

When to Use Modifier 81
  • A physician provides minimal assistant surgeon services.

  • Assistance is required for only part of the surgical procedure.

  • The procedure is eligible for assistant surgeon billing.

  • Medical necessity supports the need for an assistant surgeon.

Billing Example

A general surgeon performs a complex abdominal procedure independently but requests another physician to assist briefly during a difficult portion of the operation involving dense adhesions. The assisting physician reports the procedure code with Modifier 81 to identify the limited assistant surgeon services provided.

Documentation Tips
  • Document why assistant surgeon services were medically necessary.

  • Describe the portion of the procedure requiring assistance.

  • Clearly identify the assistant surgeon in the operative report.

  • Verify payer eligibility before submitting the claim.

Common Denial Reasons
  • Documentation does not support medical necessity.

  • The procedure is not eligible for assistant surgeon reimbursement.

  • The operative report does not identify the assistant surgeon.

  • Modifier 81 is reported instead of the appropriate assistant surgeon modifier.
